This form is a certification by a relevant professional that there is a Specific Learning Disability or other condition that is impacting on the applicant’s schoolwork. The form supplements a professional report/diagnosis which should also be included in the application. The form must be completed by an Educational Psychologist or relevant professional/specialist. NUI do not accept certification forms that are completed by General Practitioners. A Jpeg or Pdf of the form will need to be uploaded under relevant grounds.

Please note that if an applicant has a Department of Education exemption, they do not need to provide this form or a professional report but instead apply under option 4 where a copy of the cert and a School Principal Declaration Form will be required.

School Principal Declaration Form

This form is a certification provided by the school and the purpose is to attest to the information provided by the applicant in relation to either

1. their schooling history,
2. their exemption from study of Irish and/or Third language on the basis of a Department of Education Exemption
3. their exemption from the study of Irish and/or Third language on the basic of a Specific Learning Disability or other condition

The form should be completed and then signed by the school principal. A Jpeg or Pdf of the form will needed to be uploaded under relevant grounds.
